The security management measures implemented by our company are as follows:

Organizational Security Measures
  • A person responsible for the handling of personal information has been appointed.

  • The scope of employees who handle personal information and the scope of personal information handled by them are clearly defined.

  • A reporting system has been established to ensure that any violations or suspected violations of laws or internal handling rules are promptly reported to the responsible person.

  • Internal audits regarding the handling of personal information are conducted once per year (or as necessary).

Human Security Measures
  • Employees receive regular training regarding the proper handling of personal information.

  • Confidentiality obligations related to personal information are stipulated in employee confidentiality agreements signed upon hiring.

Physical Security Measures
  • Documents containing personal information are stored in locked storage areas to prevent unauthorized access.

Technical Security Measures
  • Access control is implemented for systems (terminals and data) that handle personal information, restricting access to authorized personnel and limiting the scope of accessible data.

  • Information systems handling personal information are protected against unauthorized external access and malicious software.

3. Procedures for Requests for Disclosure of Personal InformationRequests for notification of the purpose of use, disclosure, correction, addition, deletion, suspension of use, erasure, suspension of provision to third parties, as well as complaints or consultations regarding personal information, will be handled by our Personal Information Inquiry Desk.

Required Documents

(1) Personal Information Inquiry Request Form

Please fill in the following information on the designated form:

  1. Address, name, telephone number, and seal/signature of the requester

  2. Date of request

  3. Information necessary to identify the personal information to be disclosed

(2) Proof of Identity (for the individual or their representative)

If the requester is the individual:

Submit a copy of one of the following:

  • Driver’s license (front and back)

  • Passport

  • Health insurance card

  • Residence card

If the requester is a representative:

  • Power of attorney (required)

  • Seal registration certificate of the individual whose seal appears on the power of attorney

  • One form of identification for the representative

(3) Processing Fee
JPY 1,000 (only for requests for notification of purpose of use or disclosure).
Payment must be made via bank transfer to a designated account after submission.
Bank transfer fees are the responsibility of the requester.

If a request is submitted by a representative, please contact the PMS Office in advance.

Authorized representatives are limited to:

  • Legal guardians of minors or adult wards

  • Representatives authorized by the individual through a power of attorney

Upon receiving a request, we will conduct the necessary investigation and respond within two weeks in writing. To prevent unauthorized disclosure, responses will be sent via restricted delivery mail addressed exclusively to the individual.
